There are conflicting results in the literature as to whether testosterone therapy has a significant impact on these symptoms. Men who seek medical care for possible testosterone therapy often present with non-specific symptoms, such as low energy and fatigue, which can be manifestations of other conditions, such as chronic stress, chronic fatigue, and depression. The rate of remission was also higher in a statistically significant manner among dysthymic men receiving testosterone therapy (53%) compared to placebo (19%).317, 318
The best time to obtain monitoring blood tests for IM testosterone has not been definitively established. One RCT by Maggi et al. followed 715 testosterone deficient men for 12 weeks to evaluate the effects of a 2% transdermal testosterone agent on sex drive and energy. All patients had PSA and digital rectal exams every three months and biopsies annually. Six patients experienced biochemical recurrence, all of whom had intermediate- or high-risk prostate cancer. If the testosterone concentration is increased further, rather than further proliferation, the cells reduce their rate of proliferation.343, 344 This phenomenon is known as the bipolar testosterone concept. However, the saturation model introduced by Morgentaler is based on the concept that prostate cancer cells\' response to the testosterone level to which they are exposed is not linear in nature.
Unfavorable changes were reported in 2 studies 5, 25 and included increases in total cholesterol, LDL cholesterol, and triglycerides and decreases in HDL cholesterol in men evaluated as hypogonadal prior to androgen therapy. Favorable effects in 11 studies included 5–11% decreases in total cholesterol concentration and variable and inconsistent decreases in triglycerides and low-density lipoprotein (LDL) cholesterol. Serum or plasma concentrations of cholesterol fractions, triglycerides, and lipoproteins have been used as surrogate endpoints for cardiovascular risk, although they should not be mistaken for markers of cardiovascular adverse events. The improvements in exercise function appeared attributable to the response of men with baseline plasma testosterone concentration 23 Another study showed no effect of IM testosterone enanthate on ejection fraction, although there was an improvement in a Doppler-based myocardial performance index. Intramuscular (IM) testosterone treatment for 12 weeks improved exercise capacity and reduced heart failure symptom scores without identifiable effects on left ventricular size or ejection fraction (EF). There was a decreased incidence of silent MI with testosterone treatment in 1 study. The remaining eight studies evaluated treatments of 2 to 24 weeks in duration.1, 3, 10, 11, 13–17 The 3 studies that looked at time to ST-segment depression found a benefit of testosterone supplementation.1, 14, 16|Most authors with an interest in 17β-estradiol have suggested that the effectiveness of testosterone, when it has shown effectiveness, is due to aromatization to 17β-estradiol. Treatment of men with suspected or diagnosed Alzheimer disease or cognitive impairment was reported in five studies, two of which had a Jadad score above 3. A systematic review found that varicocele ligation results in significant improvement in testosterone levels in some men, with a mean improvement of approximately 100 ng/dL.|Commanders and medical boards may consider whether the therapy affects physical readiness, cognitive function, mood, or cardiovascular risk. To the extent that this increase in use of testosterone supplementation is based on anticipated improvements in cardiovascular health, sexual function, physical functioning, mood, or cognition, we suggest that it might represent therapy without adequate clinical trial support. Observational studies attributing positive health effects to testosterone may be affected by an increased likelihood of healthier men being prescribed testosterone rather than testosterone improving health. A Medicare-based study identified testosterone exposures and MI outcomes using claims data and matched testosterone-treated with untreated subjects using an empirically derived propensity score and found no increased risk. Men who were treated with testosterone had an increased risk of all-cause mortality, MI, and stroke compared to men who did not use testosterone (HR, 1.29; 95% CI, 1.05–1.58), based on a mean of 27.5 months of follow-up. A Veterans Administration study evaluated men who had undergone coronary angiography and had a total testosterone concentration (presumably plasma) less than 300 ng/dL (10.4 nM).|Ten studies evaluated subjects with HIV; 8 of those studies included subjects with HIV-wasting, 1 included subjects with abdominal obesity, and 1 did not use weight criteria. Twenty studies evaluated subjects described as hypogonadal, with 11 of those evaluating healthy subjects. Physical function was often measured by the 6-minute walk test, the time and number of steps required to walk 25 feet, and the get-up-and-go test, which evaluates the ability to rise from a chair, walk a short distance, and return to sitting. Subjects included both those defined by the authors as hypogonadal and those considered to have normal testosterone concentrations. Ten of 13 of the studies on libido or desire with a Jadad score of 4 or 5 found a benefit.|The unique pharmacokinetic profile of testosterone pellets is due to their crystalline structure, which dissolves slowly in SQ spaces. The manufacturer-recommended dosing of IM testosterone undecanoate is 750 mg administered at weeks 0, 4, and every 10 weeks thereafter. No RCTs have compared the current formulation of IM testosterone undecanoate available in the United States to other therapies. Testosterone undecanoate is the only currently available long-acting injection therapy available as a 750 mg (3 mL) preparation and must be administered in the office under supervision. However, compared to other agents, short-acting injections can result in longer times in the supra-therapeutic and sub-therapeutic ranges, which may impact overall efficacy and rates of adverse events.|It restores the circulating testosterone level to the physiological range. The most common adverse effect with patches is application site reactions, which have been historically reported in up to 60% of patients.181 Other adverse effects include pruritus, application site vesicles, and back pain.431 Compared to topical gels and solutions, the rate of transference is likely minimal. Adverse effects specific to topical preparations include application site reactions (3-16% erythema or rash), and risk of transference. There is no consistent data at this time that demonstrate that one agent achieves higher serum levels than others. Although one objective of meta-analyses is to increase study power to identify significant results, this often results in an amalgamation of studies that may have different primary and secondary endpoints, thereby reducing the reliability of the outcomes. It also highlights that treating clinicians should have specific endpoints for treatment in mind, with regular monitoring of these outcomes to assure that ongoing therapy is warranted and effective.}

Pituitary dysfunction may be a significant cause of testosterone deficiency. A survey of 120 patients who were treated for infertility at the University of Illinois-Chicago found that the incidence of testosterone deficiency was 45% in men with non-obstructive azoospermia, 42.9% in men with oligospermia, and 16.7% in men with obstructive azoospermia.159 There does appear to be a trend towards lower total testosterone and a diagnosis of ED.
